La première partie de mon parcours hospitalier en Guyane a consisté à structurer l’offre de soins, avec initialement la création du service d’Endocrinologie-Diabétologie-Nutrition en 2017. Ainsi, après l’ouverture du service j’ai pu commencer à organiser la filière de spécialité territoriale. Cette étape fondamentale au développement de la spécialité sur le territoire a été la première pierre angulaire de la mise en place de la spécialité en Guyane.

La deuxième partie de mon parcours guyanais a été le développement de l’enseignement et de la recherche clinique. En effet, j’ai pu mettre en place en parallèle de l’enseignement pour les internes et l’école d’infirmières, un Diplôme Universitaire de Diabétologie (depuis 2017), et un congrès de Diabétologie et Pathologies Métaboliques (depuis 2016). L’enseignement est également conduit au sein du service, à travers les staffs de dossiers et bibliographiques, auprès des médecins de ville avec des formations Développement Professionnel Continu dans la spécialité (agrément dans le cadre de l’endocrinologie et des pathologies métaboliques pour l’association que je préside la Diabète Guyane Obésité depuis 2020). La recherche clinique a été mise en place après le recrutement de médecins et d’assistants, notamment partagés avec les services de spécialité en Endocrinologie Diabétologie Nutrition de Paris et Lyon, me permettant de libérer le temps nécessaire à la mise en place des projets structurants dans les différentes thématiques : diabète, pathologies métaboliques et thyroïde. Les premières études de recherche clinique ont permis la description du diabète en Guyane, afin de poser les problématiques et en particulier la prise en charge des inégalités de santé et des diabètes atypiques, notamment les diabètes dits de « l’africain ». Plusieurs thèses dans le domaine de l’Endocrinologie et du Métabolisme ont donné lieu à des publications dans des revues à comité de lecture, permettant ainsi d’augmenter la visibilité de la spécialité au niveau international.

En 2019 j’ai pu mettre en place comme investigateur principal deux cohortes territoriales, sur la thématique du diabète : la cohorte CoDiAm et 2020 la cohorte CCoviDiAm. CoDiAm est un projet ambitieux de suivi longitudinal, sur deux ans, d’une cohorte de 1300 patients diabétiques, pour lesquels nous avons plus de 600 paramètres épidémiologiques, cliniques et biologiques recueillis. Nous avons la volonté de pérenniser cette cohorte en un registre du diabète en Guyane, afin d’obtenir une base de données permanentes et actualisées sur les caractéristiques du diabète en Guyane et d’en adapter la prise en charge en fonction des résultats.

La troisième étape de mon projet en Guyane est le développement de l’ensemble des trois axes déjà initiés : – poursuivre l’enseignement des internes et des étudiants en 2^e année de médecine, développer les formations inter-régionales/ nationales (avec les Antilles et la métropole) ; – développer la recherche clinique sur un double axe « métabolique et endocrinien Tropical » ; – poursuivre le développement de l’offre de soins en insistant sur le décloisonnement ville/hôpital.

Conclusion

Je suis actuellement Praticien Hospitalier au Centre Hospitalier de Cayenne, et ai mis en place le service d’Endocrinologie Diabétologie Nutrition de référence et l’équipe territoriale. J’ai intégré à 1/3 temps l’équipe de recherche INSERM 14-24 en tant que médecin chercheur (depuis 2020), et faisons partie de l’équipe de l’EA3593, EPAT : Ecosystèmes Amazoniens et Pathologie Tropicale. Je travaille avec les différents intervenants de l’enseignement universitaire et de la recherche de manière transversale dans le département afin de développer l’Endocrinologie et pathologies métaboliques « Tropicales » en Guyane, et souhaite continuer à développer les projets de recherche et d’enseignement ambitieux en collaboration avec les Antilles et la France Hexagonale. L’habilitation à diriger la recherche est un des prérequis important pour me permettre de poursuivre la voie universitaire et notamment une candidature à un poste de Professeure des Universités, voie d’excellence qui permettrait de participer activement à la construction du futur CHU de Guyane 2025.

The first part of my hospital career in French Guiana consisted of structuring the care offer, initially with the creation of the Endocrinology-Diabetology-Nutrition service in 2017. Thus, after the opening of the service I was able to start organizing the territorial specialty pathway. This fundamental step to the development of the specialty in the territory was the first cornerstone of the establishment of the specialty in French Guiana.

The second part of my Guyanese career was the development of teaching and clinical research. Indeed, I was able to set up in parallel to the teaching for interns and the nursing school, a University Diploma of Diabetology (since 2017), and a congress of Diabetology and Metabolic Pathologies (since 2016). Teaching is also conducted within the department, through staffs of files and bibliographic, with city doctors with training Continuous Professional Development in the specialty (accreditation in the context of endocrinology and metabolic pathologies for the association that I preside the Diabetes Guyana Obesity since 2020). Clinical research was set up after the recruitment of doctors and assistants, notably shared with the Endocrinology Diabetology Nutrition departments of Paris and Lyon, allowing me to free up the time necessary to set up structuring projects in the different themes: diabetes, metabolic pathologies and thyroid. The first clinical research studies allowed the description of diabetes in French Guyana, in order to pose the problems and in particular the management of health inequalities and atypical diabetes, notably the so-called « African » diabetes. Several theses in the field of Endocrinology and Metabolism have resulted in publications in peer-reviewed journals, thus increasing the visibility of the specialty at the international level.

In 2019 I was able to set up as principal investigator two territorial cohorts, on the theme of diabetes: the CoDiAm cohort and 2020 the CCoviDiAm cohort. CoDiAm is an ambitious project of longitudinal follow-up, over two years, of a cohort of 1300 diabetic patients, for whom we have collected more than 600 epidemiological, clinical and biological parameters. We want to perpetuate this cohort into a diabetes registry in French Guyana, in order to obtain a permanent and updated database on the characteristics of diabetes in French Guyana and to adapt the management according to the results.

The third step of my project in French Guiana is the development of all three axes already initiated: – to continue the teaching of interns and students in their 2nd year of medicine, to develop inter-regional/national training (with the West Indies and metropolitan France); – to develop clinical research on a double axis « metabolic and endocrine Tropical »; – to continue the development of the care offer by insisting on the decompartmentalization city/hospital.

Conclusion

I am currently a Hospital Practitioner at the Cayenne Hospital, and have set up the Endocrinology Diabetology Nutrition referral service and the territorial team. I have joined the INSERM 14-24 research team as a research physician (since 2020), and am part of the EA3593 team, EPAT: Ecosystèmes Amazoniens et Pathologie Tropicale. I work with the different stakeholders of university teaching and research in a transversal way in the department in order to develop Endocrinology and « Tropical » metabolic pathologies in French Guiana, and wish to continue to develop ambitious research and teaching projects in collaboration with the West Indies and France Hexagonale. The habilitation to direct research is one of the important prerequisites to allow me to pursue the academic path and in particular a candidacy for a position of University Professor, a path of excellence that would allow me to actively participate in the construction of the future University Hospital of Guyana 2025.
